This session focuses attendees on capturing HCCs and how they can incorporate claims data to identify gaps in HCC and risk adjustment scores. Practices can learn how to proactively manage HCCs as a financial tool, as well as a quality measure. This session will help practices explore the relationship between HCC risk scoring and maximizing payer reimbursement for quality incentives.
